Description & Services
Description of service(s) The Communicable Disease team consists of Public Health Nurses and a Public Health Nurse Practitioner. In collaboration with the Medical Officer of Health, the team investigates and assists with the control of: 
~Diseases spread by direct contact and respiratory routes 
~Sexually Transmitted and Bloodborne Infections  
~Vaccine preventable Diseases 
 
The Communicable Disease team: 
~Provide information on childhood diseases; 
~Provide information on infectious diseases; 
~Form community partnerships to reduce spread of communicable diseases; 
~Communicate with primary care providers in follow-up to specific communicable diseases; 
~Inform contacts, as required, that they have been exposed to a particular disease; 
~Provide education and instruction to individuals who have been exposed to a communicable disease; 
~Conduct surveillance and monitor trends in reportable communicable diseases in our Zone 
~Provide Medical Surveillance for certain individuals identified by Citizenship and Immigration Canada 
~Provide basic health screening, referrals and interim health care for new Refugees
