Description of service(s) Assessments Plus is a client centered company that provides the highest quality of home health care professionals and case management. We promote dignity and independence in aging while offering frequent home visits and communication with family members to ease the burden of worry. 
 
Assessments Plus works closely with families, physicians, and medical professionnals to allow seniors to remain in their home. Services include: 
 
Home Support Services 
General Nursing (RN, LPN) 
Palliative Care Services 
Companion Sitters 
Respite Care 
House Keeping Service, Laundry, Meal Preparation, Medication Reminders, Assistance with Daily Living.  
 
Footcare Services 
 
Registered with the Department of Social Development, Blue Cross and Veteran's Affairs.
Hours Monday-Thursday 8 am-4 pm, Friday 8 am- noon 
24 hour Emergency 506-721-7728
